Welcome to The Tridas Center. Thank you for choosing our child development and behavior services. Please review the following information to expedite making an appointment in our Center.

STEP 1. Call 813-876-8316 x 3 and make an appointment.

In order to schedule the first appointment you will need to call 813-876-8316 x3 to speak with our appointment coordinator. At this time you will be asked to provide some basic demographic information, briefly describe the concerns for your child and set an appointment date and time.  Once you have a specific appointment date you can then follow the steps listed below to complete the web-based questionnaires that we will need before your child’s first visit.

STEP 2. Set up online questionnaire registration.

• The next step will be to go online to complete a self registration and begin the setup process for your child’s secure online Patient Questionnaire below. 

      The questionnaires are an essential step in your child’s assessment and should be completed by the parents and teachers no later than 10 days prior to the first evaluation appointment.  We assemble the parent’s and teacher’s responses to the online interview into a report and place it in the child’s file for the doctor’s review prior to the appointment.

There is a one-time non-refundable charge of $50.00 to process the interview data completed online.  In addition, our practice’s financial & cancellation policies will be provided for you to sign electronically prior to the first visit. 

• Once you have clicked to start the questionnaire registration process you will be asked to select your method of payment.  Once the transaction is completed you will receive confirmation on the screen.  This will provide you with all the necessary information to access our secure web based questionnaires. These will include the following:

1. Your child’s personal Tridas Center email address and password required to access the questionnaires
2. All the PIN numbers which will ensure confidential access to the proper interview (parent or teacher)
3. Instructions on how to answer the questionnaires

In addition you will receive a copy of the financial policy that you signed electronically.
