The Center uses the information we gather from you to produce the Educational Evaluation for your child. Patients, their teachers and families will be using a secure link to The Center’s website to provide the information required by our professional staff to accurately evaluate the needs of your child. We will disclose your Personal Information when you direct or give us permission to do so, when we are required by law to do so, or when we reasonably believe that disclosure is necessary to protect The Center or you or third parties from a threat of imminent harm, such as through suspected fraudulent or criminal activities. We also may disclose your Personal Information when otherwise permitted by applicable privacy laws such as, for example, when disclosure is needed to enforce our rights arising out of any agreement, transaction or relationship with you. We may collect the following types of Personal Information from you when you visit the website: e-mail address. The website also allows you to complete evaluative questionnaires regarding your child. Personal Information collection may take place through our Internet Web Site or through paper forms/applications provided to you by a staff member of The Tridas Center.
