Financial Policy / Fee Schedule

The following schedule is effective January 1st 2006, and may be changed without notice.

Cost of evaluation: The fees charged at the Tridas Center for Child Development are based on time scheduled for each of our professional staff members on your child’s behalf including the interpretation and review of tests and preparation of report. The charge for an initial visit by our doctors is $515.00.  The average time spent during the first visit is between 1½ to 2 hours (contact time with you and your child)   Afterwards, the physician will score and interpret all tests administered, may consult with other professionals and will prepare an extensive report.  The second visit or feedback session lasts approximately 1 to 1½ hours.  The charge for this appointment is $275.00.  There is a $50.00 non-refundable processing fee that must be paid when you register your child on our confidential website.  This fee will cover the cost of processing the parent, teacher and student (if appropriate) questionnaires and the data analysis that is done prior to your first visit to our center.   

Method of payment: Full payment is expected at the conclusion of each visit except for the $50.00 non-refundable processing fee which is paid online when you register prior to accessing the questionnaires on our secure website.  We will make every effort to provide you with the basic documentation necessary for you to submit a claim to your insurance. We do not accept or process insurance payment but we accept VISA, MasterCard and American Express as an alternative.

Forms to be completed: You will be provided with a financial responsibility agreement and a patient information form in your patient information packet that must be signed (electronically or on paper) and returned to our center prior to your first visit.  We also provide on-line questionnaires with the necessary passwords and Personal Identification Numbers (PIN) for you, your child and the teachers that will need to be completed through our secure website prior to the first visit.   These questionnaires will enable us to gather  information about your child that is essential for our doctors to formulate a proper diagnosis and management plan.  Because this data is critical to the evaluation, the appointment cannot be completed without it.  If the information cannot be submitted electronically, there will be an additional $50.00 surcharge to enter the data manually.

Cancellation policy: You will be charged $250 if you cancel a New Patient Evaluation without adequate notice. New patient evaluations and parent feedback visits must be cancelled no later than 1 business day (24 hours, or Friday morning if your child’s appointment is on a Monday) prior to the visit.   We do not “double book” at The Tridas Center for Child Development.  Understand that we will schedule two and a half to three hours of our professionals’ time to be spent with you and your child for a New Patient Evaluation that cannot be rescheduled without adequate notice.

Below please find the procedure codes for the charges at our center.  You can use this information to check with your insurance carrier for reimbursement rates.

Please note that other tests may be advised after the initial visit such as speech or psychoeducational testing (IQ, processing, and achievement) are not included in the rates quoted. These tests will be performed by other licensed professionals.
